Test the Check Box: After inserting a check box, test it in your document. Switch to “Design Mode” in the Developer tab to make adjustments, and switch back to preview the user interaction.

Select the "Check Box Content Control" icon, which will insert the control into the Word document. Edit the properties of the checkbox control, as necessary. Click the checkbox.
